We know life as a parent or caregiver can feel frustrating 
& overwhelming at times.

If you’re a parent or caregiver, you know that your role brings many distinct joys and privileges. You get to be present with your loved one in a unique way as they craft a fulfilling life.

At the same time, there are days when it can be exceptionally difficult and isolating if you are surrounded by people who don’t understand the challenges you navigate on a regular basis.

At Hopewell Center, we recognize the importance of having a community that supports you and your loved ones. Through professional and relational care, we offer the resources and services necessary for your loved one to thrive and for you to feel equipped to help them get there.

Children

A ball is just a ball until a child plays with it and turns it into a valuable learning tool.

It might sound simple, but playtime is an important aspect of child development, and some children need a little extra help along the way. Each of Hopewell's programs provide ample time for kids to play, learn, and grow, offering both individualized attention and support for the foundation parents have built with their children..

Through weekly home visits, parent interaction, socialization groups, and therapy services,
we can minimize the effect of a developmental delay on a young child, or help a family become more self-sufficient via a range of programs:

  • First Steps-Developmental Therapy
  • First Steps-Physical Therapy
  • First Steps-Occupational Therapy
  • First Steps-Speech Therapy

Eligibility - Homestart Early Learning - birth to 36 months of age, and demonstrated delay or "at risk" status as defined in the Division of Disability and Rehabilitative Service's First Steps website.

Employment

Work helps us create our future, and it also helps us shape our place in the community.

Hopewell plays a key role in helping people with disabilities gain independence through employment, and our Community Employment Services Program matches motivated workers with employers by finding jobs that are a perfect fit for an individual’s skills and interests.

Employment isn’t just about expertise, however. It also takes community involvement, so Hopewell offers the necessary support to meet the needs of both employees and employers, creating successful partnerships that benefit the entire community through the following services:

  • Supported Employment Follow Along

Eligibility - Community Employment - Age 18 or older, and eligible for third-party funding (Office of Vocational Rehabilitation) or an agreement to self-pay and support needs consistent with community employment.

Life Skills

We live in a world of unspoken rules.

Whether you’re following a recipe, shopping, making friends, or making music, the freedom of fitting in is found through knowledge. Learning how to interact with the world begins with a choice, but it continues with the support of a community helping people navigate what they choose to do, who they choose to be, and where they choose to go.

At Hopewell, we offer Habilitation training in Life Skills tailored to each person’s dreams, and we share the resources necessary to transform those dreams into goals.

Eligibility - Supported Living - Diagnosed as developmentally disabled, and an approved plan of care to receive Medicaid Waiver services, approved level of support that meets level of need, and eligible for work training and/or community employment.

Residental

An open door is an open invitation. It beckons us to look inside and feel welcome. When we open doors to those with disabilities, we create space for a new way of life.

At Hopewell Center, our residential services provide a wide spectrum of support, ranging from around-the-clock assistance to weekly visits. Both children and adults who participate in our programs develop the skills they need for daily living, and families receive the support they need to focus on their responsibilities. Additionally, we also connect people to community resources and activities, all while situated in any of the following residential programs:

  • Group Homes
  • Home and Community Based Waiver including
  • Residential Habilitation (up to 24 hr supervision)
  • Community Habilitation
  • Wellness Coordination
  • Structured Family Caregiving
  • Respite
  • Personal Assistance and Care
  • Remote Supports
  • Community Transition Services

Eligibility - Group Home - Age 18 years or older, and diagnosed as developmentally disabled with functioning levels similar to other residents; eligible for work training and/or community employment, Medicaid eligibility, TB & Hep B screen.

Day Services

Not everyone needs the vocational services that Hopewell Center has historically specialized in, which is why we offer Life Unlimited day services.

This program is designed to help teams of 4-6 individuals explore and identify their dreams and desires, whether that means sculpting something, writing a story, or acting in a play. These sessions meet 3 ½ hours a day, Monday through Friday, and facilitators walk them through interpersonal skills in planning, team building, communication, brainstorming, negotiation, and self expression.

Life Unlimited is currently offered in two forms:

  • Group Habilitation, both facility-based and community-based
  • Individual Habilitation,both facility-based and community-based

Eligibility and apply for services - Day Services - Age 18 years or older, Medicaid or Medicaid Waiver eligibility.

Children Services

Apply online through First Steps.

Each program has unique eligibility criteria:

  • Homestart Early Learning (birth to 36 months of age)

18+ Years

Adult Services

Apply online through Bureau of Developmental Disability Services.

FAQs

How much does it cost to receive Hopewell Center services?

Once you have been approved to receive services, the majority of services provided by Hopewell Center are funded through the Home and Community-Based Services waiver. Some services provided through the First Steps program may have small co-pays based on family income as determined by the Indiana state government.

Can I choose which services I want to receive?

Yes! Once you have been approved for services, you will be able to select which programs fit your needs and goals.

How can Hopewell help me or my family?

Life may have many challenges, but help comes in many forms at Hopewell, including:

  • Helping a child learn to sit-up, crawl, stand, and walk
  • Listening to a family’s frustrations, fears, and needs
  • Teaching nutrition basics
  • Teaching skills for finding and retaining employment
  • Helping people build meaningful connections in the community
  • Identifying what people need in order to experience an independent life

How are staff certified?

Each staff position has distinct requirements, ranging from a clean driving record to CPR certification, supervisory experience, specific education or training, and demonstrated care and understanding of persons with special needs. We encourage you to review the qualifications of any staff position by contacting the Executive Director or their designee, and they’ll schedule an appointment to review the essential functions of a position, hiring qualifications, and on-going staff training. We welcome feedback on qualifications and role expectations.

How do I know if I'm eligible?

Each program has different eligibility criteria:

I feel overwhelmed. Where do I even start?

Call us! We want to help you make informed decisions, so we will put you in touch with qualified professionals who can help you form a plan. Hopewell staff will assess your needs and identify appropriate services. If your need is beyond the purview of our services, we'll put you in touch with someone who can offer support.

There are so many options out there. Who should I choose?

There are many service providers ready to provide assistance to you and your family, and it can be difficult to determine the best option. With that in mind, consider asking the following questions:

  • Does the provider have a broad spectrum of services from which to choose?
  • Is the provider a leader in their field? Is the provider considered an expert to the community, higher education, industry associations, and state agencies?
  • Does the provider receive accreditation from national performance standard associations?
  • Does the provider regularly ask their customers if they're satisfied, and share the results publicly?
  • Is the provider ready to suggest competitors in an effort to be sure that you are satisfied with your services?

How much will this cost?

Most services at Hopewell are funded in whole or in part through partnerships with various State Agencies, so costs are kept at a minimum.

How do I sign up?

To sign up for Adult Services, start by applying online for funding from the Bureau of Developmental Disability Services here.

To sign up for Children's Services for children up to three years, start by applying online for First Steps funding with First Steps.
